MAKING APPOINTMENT
Appointments are available between 8:30am – 5pm and can be made by phoning the clinic or coming in during these hours. Each appointment is for one (1) person only. If you require a longer consultation to discuss more than one issue or a procedure, please advise the reception.

If you have an emergency, please advise the reception to enable appropriate care or call: 000.

Please cancel appointments at least 3hrs prior to the scheduled time, in order to enable the appointment time to be re-filled.
Nursing services are available as co-ordinated by your doctor.

BILLING
This practice is a mixed billing facility.
Pre-employment assessments, Driving Medical, Medical reports etc. Some treatments/procedures also incur out of pocket expenses.
Please check with the reception.

SCRIPT AND REFERRAL LETTERS
It is important to see your doctor for scripts and referrals.
If your doctor write either without consultation, it may incur a cost and a delay of up to 48hrs. Please note, referrals cannot be backdated, so ensure your referral is current prior to seeing the Specialist.

TEST RESULT

You are required to book an appointment for your test results. They will NOT be given over the phone for privacy reason. Please contact the clinic after 3 days to check if your results are back and then make appointment. Alternatively the Nurse may contact you to advise you to see the doctor for review. RECALL / REMINDER SYSTEM
Our practice is committed to providing continuing and preventative health care. We operate a recall/ reminder system.
If you do not wish to participate in this system please advise reception staff.

TELEPHONE CALLS
You may leave a message for the doctor, as only the URGENT calls, will be transferred to the doctor.

EMAIL

Patients are advised not use email to contact the practice, as we cannot guarantee confidentiality of information transferred via email.

PARKING
Parking is available in the car park opposite the Clinic, car parks are also available on William Street, or Bank Street where easy access is available.
If a wheelchair is required please ask reception for assistance

If you require an interpreter, please let the reception know. Alternatively Contact

PATIENT PRIVACY AND PERSONAL HEALTH INFORMATION
Your medical record is a private and confidential document and only available to authorised members of staff.
By signing the registration and medical record consent form, you are agreeing to the terms that guide the Privacy Act 2000.
If you wish to access your information in electronic health record, please advise our authorised staff who will facilitate this for you. If you wish to transfer your records from your previous Clinic, please ask reception. Some Practices charge an administration fee for this.

PATHOLOGY
Dorevitch Pathology is onsite and offers variety of services and test.
